ABSTRACT

Background: The purpose of this study is to update results of Portuguese's Report Card on Physical activity (PA) for Children and Adolescents. Methods: The grades were assigned by results derived from the PA and Fitness in Portugal 2021 Portuguese Report Card and corresponds to the third report for the Portuguese children and adolescents. It includes indicators of PA and sedentary behavior (SB) that are common to the GLOBAL matrix 4.0: Overall Physical Activity, Organized Sport and Physical Activity, Active Play, Active Transportation, Sedentary Behaviors, Family and Peers, School, Community and the Environment, Government and Physical Fitness. The search focused on published national evidence/data sources (academia, NGO, governmental) from end 2018 onwards excluding data obtained during the covid-19 pandemic. Results: The grades were assigned as follows: Overall PA (D-), Organized Sport Participation (C-), Active Play (D+), Active Transportation (D-), Sedentary behaviors (C+), Physical Fitness (C), Family and Peers (B), School (A), Community and Environment (B), and Government (B). Conclusion: In line with previous Portuguese Report Cards, a large proportion of Portuguese children and adolescents are not sufficiently active nor fit enough setting urgency for effective strategies. Particular attention should be given to Active play, Active transport and Organized Sports Participation has their grades have decreased. Some actions in selected indicators as Governmental and policy seems promising however results weren't seen yet. Despite the strong support of schools with mandatory curricula in PE no correspondent change is observed in fitness or PA, so more research is needed to find why.

ABSTRACT

ABSTRACT Objective: to describe the development of a virtual assistant as a potential tool for health co-production in coping with COVID-19. Method: this is an applied technological production research study developed in March and April 2020 in five stages: 1) literature review, 2) content definition, 3) elaboration of the dialog, 4) test of the prototype, and 5) integration with the social media page. Results: the literature review gathered diverse scientific evidence about the disease based on the Brazilian Ministry of Health publications and by consulting scientific articles. The content was built from the questions most asked by the population, in March 2020, evidenced by Google Trends, in which the following topics emerged: concept of the disease, prevention means, transmission of the disease, main symptoms, treatment modalities, and doubts. Elaboration of the dialog was based on Natural Language Processing, intentions, entities and dialog structure. The prototype was tested in a laboratory with a small number of user computers on a local network to verify the functionality of the set of apps, technical and visual errors in the dialog, and whether the answers provided were in accordance with the user's question, answering the questions correctly and integrated into Facebook. Conclusion: the virtual assistant proved to be a health education tool with potential to combat "Fake News". It also represents a patient-centered form of health communication that favors the strengthening of the bond and interaction between health professionals and patients, promoting co-production in health.


RESUMEN Objetivo: describir el desarrollo de un asistente virtual como posible herramienta para la co-producción en salud a fin de hacer frente al COVID-19. Método: trabajo de investigación aplicado de producción tecnológica, desarrollado en marzo y abril de 2020 en cinco etapas: 1) revisión de la literatura, 2) definición del contenido, 3) elaboración del diálogo, 4) prueba del prototipo y 5) integración con la página web del medio social. Resultados: en la revisión de la literatura se reunieron evidencias científicas sobre la enfermedad a partir de las publicaciones del Ministerio de Salud de Brasil, al igual que sobre la base de consultas en artículos científicos. El contenido se elaboró a partir de las preguntas más frecuentes de la población, en marzo de 2020, puestas en evidencia por medio de Google Trends, donde surgieron los siguientes temas: concepto de la enfermedad, formas de prevención, transmisión de la enfermedad, principales síntomas, modalidades de tratamiento y dudas. La elaboración del diálogo se basó en el Procesamiento de Lenguaje Natural, en intenciones, en entidades y en la estructura del diálogo. El prototipo se puso a prueba en un laboratorio con una cantidad reducida de computadoras usuario en una red local para verificar la funcionalidad del conjunto de aplicaciones, errores técnicos y visuales acerca del diálogo, y si las respuestas proporcionadas estaban de acuerdo con la pregunta del usuario, respondiendo correctamente los interrogantes e integrado a Facebook. Conclusión: el asistente virtual demostró ser una herramienta de educación en salud con potencial para combatir Fake News. También representa una forma de comunicación en salud centrada en el paciente que favorece el fortalecimiento del vínculo y la interacción entre profesionales de la salud y pacientes, promoviendo así la coproducción en salud.

Método: trata-se de uma pesquisa aplicada de produção tecnológica, desenvolvida nos meses de março e abril de 2020 em cinco etapas: 1) revisão de literatura, 2) definição de conteúdo, 3) construção do diálogo, 4) teste do protótipo e 5) integração com página de mídia social. Resultados: a revisão de literatura reuniu evidências científicas sobre a doença a partir das publicações do Ministério da Saúde, no Brasil, e de consultas em artigos científicos. O conteúdo foi construído a partir das perguntas mais realizadas pela população, em março de 2020, evidenciadas por meio do Google Trends, em que emergiram os seguintes temas: conceito da doença, formas de prevenção, transmissão da doença, principais sintomas, formas de tratamento e dúvidas. A construção do diálogo foi baseada em Processamento de Linguagem Natural, intenções, entidades e estrutura de diálogo. O protótipo foi testado em laboratório com um número reduzido de computadores usuários em uma rede local para verificar a funcionalidade do conjunto de aplicações, erros técnicos e visuais acerca do diálogo e se as respostas fornecidas estavam de acordo com a pergunta do usuário, respondendo de forma correta os questionamentos e integrado ao Facebook. Conclusão: o assistente virtual mostrou-se uma ferramenta de educação em saúde e com potencial para combater fake news. Também representa uma forma de comunicação em saúde centrada no paciente, que favorece o fortalecimento de vínculo e interação entre profissionais de saúde e pacientes, promovendo a coprodução em saúde.

to investigate factors associated with the SARS-CoV-2 infection among health professionals from university hospitals. a multicenter, mixed approach study with concomitant incorporated strategy, carried out with 559 professionals in the quantitative stage, and 599 in the qualitative stage. Four data collection instruments were used, applied by means of an electronic form. The quantitative analysis was performed with descriptive and inferential statistics and the qualitative data were processed by means of content analysis. the factors associated with the infection were as follows: performance of the RT-PCR test (p<0.001) and units offering care to COVID-19 patients (p=0.028). Having symptoms increased 5.63 times the prevalence of infection and adhering to social distancing most of the time in private life reduced it by 53.9%. The qualitative data evidenced difficulties faced by the professionals: scarcity and low quality of Personal Protective Equipment, work overload, physical distancing at work, inadequate processes and routines and lack of a mass screening and testing policy. the factors associated with the SARS-CoV-2 infection among health professionals were mostly related to occupational issues.

Fueron utilizados cuatro instrumentos de recolección de datos, aplicados a través un formulario electrónico. El análisis cuantitativo se realizó mediante estadística descriptiva e inferencial y los datos cualitativos mediante análisis de contenido los factores asociados a la infección fueron: realización de la prueba "RT-PCR" (p<0,001) y unidades que atienden a pacientes con COVID-19 (p=0,028). Tener síntomas aumentó la prevalencia de infección 5,63 veces y cumplir la mayor parte del tiempo con el distanciamiento social en la vida privada la redujo un 53,9%. Los datos cualitativos mostraron las dificultades que enfrentaron los profesionales: escasez y baja calidad de equipos de protección personal, sobrecarga de trabajo, distanciamiento físico en el trabajo, procesos y rutinas inadecuados y la ausencia de una política de triage y testeo masivos. los factores asociados a la infección por SARS-CoV-2 en los profesionales de la salud se relacionaron mayormente con cuestiones laborales.

(2) La alta adhesión a las precauciones estándar y al distanciamiento no disminuyeron los casos positivos. (3) La baja calidad de los EPP y las fallas en el triage dificultaron la protección en el trabajo. (4) La infraestructura hospitalaria no favoreció el distanciamiento físico entre los equipos.

Ter sintomas aumentou em 5,63 vezes a prevalência de infeção e aderir ao distanciamento social na maior parte do tempo na vida particular reduziu em 53,9%. Dados qualitativos evidenciaram dificuldades enfrentadas pelos profissionais: escassez e baixa qualidade de equipamentos de proteção individual, sobrecarga de trabalho, distanciamento físico no trabalho, processos e rotinas inadequadas e ausência de uma política de triagem e testagem em massa. os fatores associados à infecção por SARS-CoV-2 entre profissionais da saúde foram em sua maioria relacionados a questões ocupacionais.

ABSTRACT

The COVID-19 pandemic has impacted the mental health of the youngest, worsening their emotional well-being. The demand for care in psychiatric emergencies may indirectly reflect the mental health state of children and adolescents and the emotional consequences of the pandemic. Moreover, suicidality can be considered a marker of severity in this population group. Therefore, we have aimed to longitudinally describe the number of children and adolescents attended in the psychiatry emergency department due to suicidal ideation or attempts and, to explore differences in suicidality according to gender and age. A retrospective study was carried out in the University Hospital of San Juan, Alicante, Spain, from 01.01.2018 to 31.12.2021. A total of 138 participants under 18 years requesting psychiatric care due to suicidal ideation or attempts were included. The sample was composed by 35% of males and the mean age was 14.8 years old (SD = 2.2). The number of cases per year range from 10 in 2018 to 88 in 2021. Attendances were significantly higher between 2021 and the three previous years. Besides, the number of attentions registered in the last 9 months of 2021 equals those that occurred in the entire previous period. Most of the cases were girls and middle adolescents. Suicide ideation or attempts have skyrocketed in children and adolescents. This alarming increase presents a one-year lag peak from the COVID-19 outbreak and continues until the end of 2021. Girls and those over 12 years have been identified as risk groups to present suicidal ideation or attempts.

OBJETIVO: Descrever estratégias desenvolvidas para o enfrentamento da pandemia de COVID-19 em hospitais universitários. MÉTODO: Pesquisa descritiva, com abordagem quanti-qualitativa, desenvolvida a partir de um survey online com 104 enfermeiros de três hospitais universitários brasileiros. O processamento dos dados foi realizado por meio de análise textual com auxílio do software IRAMUTEQ®. RESULTADOS: Foram obtidas seis classes semânticas que representam as principais estratégias desenvolvidas para o enfrentamento da pandemia de COVID-19 no ambiente hospitalar: 1) Fornecimento de Equipamentos de Proteção Individual (17,2%); 2) Capacitações da equipe para o cuidado (18,8%); 3) Treinamento da equipe de apoio (15,6%); 4) Aquisição de equipamentos com boa qualidade (14,1%); 5) Definição de fluxos institucionais (20,5%) e 6) Promoção de apoio psicológico (14,1%). CONCLUSÃO: As estratégias elencadas pelos enfermeiros têm contribuído para a qualidade da assistência prestada aos pacientes, bem como a manutenção da saúde do trabalhador.


OBJECTIVE: To describe strategies developed to cope with the COVID-19 pandemic in university hospitals. METHOD: A descriptive research study with a quantitative and qualitative approach, developed from an online survey with 104 nurses from three Brazilian university hospitals. Data processing was performed through textual analysis with the aid of the IRAMUTEQ® software. RESULTS: Six semantic classes were obtained that represent the main strategies developed to cope with the COVID-19 pandemic in the hospital environment: 1) Provision of Personal Protective Equipment (17.2%); 2) Training of the team for the care to be provided (18.8%); 3) Training of the support team (15.6%); 4) Acquisition of good quality equipment (14.1%); 5) Definition of institutional flows (20.5%); and 6) Promotion of psychological support (14.1%). CONCLUSION: The strategies listed by the nurses have contributed to the quality of the care provided to the patients, as well as to preserving the workers' health.

ABSTRACT

Recent global events such as COVID-19 pandemic amid rising rates of chronic lung diseases highlight the need for safer, simpler, and more available treatments for respiratory failure, with increasing interest in extracorporeal membrane oxygenation (ECMO). A key factor limiting use of this technology is the complexity of the blood circuit, resulting in clotting and bleeding and necessitating treatment in specialized care centers. Microfluidic oxygenators represent a promising potential solution, but have not reached the scale or performance required for comparison with conventional hollow fiber membrane oxygenators (HFMOs). Here the development and demonstration of the first microfluidic respiratory assist device at a clinical scale is reported, demonstrating efficient oxygen transfer at blood flow rates of 750 mL min⁻1 , the highest ever reported for a microfluidic device. The central innovation of this technology is a fully 3D branching network of blood channels mimicking key features of the physiological microcirculation by avoiding anomalous blood flows that lead to thrombus formation and blood damage in conventional oxygenators. Low, stable blood pressure drop, low hemolysis, and consistent oxygen transfer, in 24-hour pilot large animal experiments are demonstrated - a key step toward translation of this technology to the clinic for treatment of a range of lung diseases.

ABSTRACT

In this work, computational chemistry methods were used to study a silicon nanotube (Si192H16) as possible virucidal activity against SARS-CoV-2. This virus is responsible for the COVID-19 disease. DFT calculations showed that the structural parameters of the Si192H16 nanotube are in agreement with the theoretical/experimental parameters reported in the literature. The low energy gap value (0.29 eV) shows that this nanotube is a semiconductor and exhibits high reactivity. For nanomaterials to be used as virucides, they need to have high reactivity and high inhibition constant values. Therefore, the adsorption of 3O2 and H2O on the surface of Si192H16 (Si192H16@O2-H2O) was performed. In this process, the formation and activation energies were -51.63 and 16.62 kcal/mol, respectively. Molecular docking calculations showed that the Si192H16 and Si192H16@O2H-OH nanotubes bind favorably on the receptor-binding domain of the SARS-CoV-2 spike protein with binding energy of -11.83 (Ki = 2.13 nM) and -11.13 (Ki = 6.99 nM) kcal/mol, respectively. Overall, the results obtained herein indicate that the Si192H16 nanotube is a potential candidate to be used against COVID-19 from reactivity process and/or steric impediment in the S-protein.Communicated by Ramaswamy H. ABSTRACT

PURPOSE: To assess the impacts of 28 months of detraining imposed by the COVID-19 pandemic on the functional fitness of older women practicing functional or concurrent training. MATERIAL AND METHODS: A clinical trial was conducted with 16 weeks of intervention and 28 months of detraining imposed by the COVID-19 pandemic. Ninety-five participants were allocated to functional training (FT - 32), concurrent training (CT - 31), or the control group (CG - 32). All the dependent variables were measured pre-training, post-training, and after the detraining period. The functional fitness was assessed by the put on and take off a t-shirt (PTS - upper limbs), timed up and go (TUG - dynamic balance), five times sit-to-stand (FTSST - lower-limb muscle power), gallon-jug shelf-transfer (GJST - global function), and 10 m walk (W10 m - walking ability) tests. RESULTS: Sixty-three women remained after the detraining period, twenty-four in the FT (66.0 ± 3.8 years), twenty in the CT (65.0 ± 4.3 years), and nineteen in the CG (69.6 ± 5.9 years). Comparing the detraining vs. the pre-intervention, the FT and CT showed a smaller reduction than CG for the PTS (ßFT = -2.296; ßCT = -1.914), timed up and go (ßFT = -0.705; ßCT = -0.600), five times sit-to-stand (ßFT = -1.970; ßCT = -2.970), gallon-jug shelf-transfer (ßFT = -1.512; ßCT = -1.003), without differences in the 10 m walk. Also, the concurrent training showed a smaller reduction than the functional training in the FTSST (ß = 1.000). CONCLUSION: Even after a long detraining period imposed by the COVID-19 pandemic, FT and CT practices were effective strategies for maintaining the functional fitness of older women.

ABSTRACT

OBJECTIVE: To characterize the cognitive profile following COVID-19 infection and its possible association to clinical symptoms, emotional disturbance, biomarkers, and disease severity. METHODS: This was a single-center cross-sectional cohort study. Subjects between 20- and 60-year old with confirmed COVID-19 infection were included. Evaluation was performed between April 2020 and July 2021. Patients with previous cognitive impairment and other neurological or severe psychiatric disorders were excluded. Demographic and laboratory data were extracted from the medical records. RESULTS: Altogether 200 patients were included, 85 subjects were female (42.3%), and mean age was 49.12 years (SD: 7.84). Patients were classified into four groups: nonhospitalized (NH, n = 21), hospitalized without intensive care unit (ICU) nor oxygen therapy (HOSP, n = 42), hospitalized without ICU but with oxygen therapy (OXY, n = 107), and ICU (ICU, n = 31) patients. NH group was younger (p = .026). No significant differences were found in any test performed attending severity of illness (p > .05). A total of 55 patients reported subjective cognitive complaints (SCC). Subjects with neurological symptoms (NS) performed worse in trail making test B (p = .013), digits backwards (p = .006), letter&numbers (p = .002), symbol digit modalities test (p = .016), and Stroop color (p = .010) tests. CONCLUSIONS: OXY patients and females referred more SCC associated with symptoms of anxiety and depression. Objective cognitive performance was unrelated to SCC. No cognitive impairment was found regarding the severity of COVID-19 infection. Results suggest that NS such as headache, anosmia, and dysgeusia during infection were a risk factor for later cognitive deficits. Tests assessing attention, processing speed, and executive function were the most sensitive in detecting cognitive changes in these patients.

The SARS-CoV2 promotes dysregulation of Renin-Angiotensin-Aldosterone. The result is excessive retention of water, producing a state of noxious hypervolemia. Consequently, in COVID-19 injury lung is pulmonary edema. Our report is a case-control study, retrospective. We included 116 patients with moderate-severe COVID-19 lung injury. A total of 58 patients received standard care (Control group). A total of 58 patients received a standard treatment with a more negative fluid balance (NEGBAL group), consisting of hydric restriction and diuretics. Analyzing the mortality of the population studied, it was observed that the NEGBAL group had lower mortality than the Control group, p = 0.001. Compared with Controls, the NEGBAL group had significantly fewer days of hospital stay (p < 0.001), fewer days of ICU stay (p < 0.001), and fewer days of IMV (p < 0.001). The regressive analysis between PaO2/FiO2BAL and NEGBAL demonstrated correlation (p = 0.04). Compared with Controls, the NEGBAL group showed significant progressive improvement in PaO2/FiO2 (p < 0.001), CT score (p < 0.001). The multivariate model, the vaccination variables, and linear trends resulted in p = 0.671 and quadratic trends p = 0.723, whilst the accumulated fluid balance is p < 0.001. Although the study has limitations, the promising results encourage more research on this different therapeutic approach, since in our research it decreases mortality.

Método: trata-se de um estudo exploratório, descritivo, com abordagem qualitativa, realizado com dez enfermeiros das Estratégias de Saúde da Família de Murici- Alagoas. Os dados foram obtidos a partir de entrevista semiestruturada, submetidos à técnica de Análise de Conteúdo de Minayo. O referencial teórico utilizado foi a Teoria da adaptação de Callista Roy. Resultados: o medo do desconhecido, a necessidade de enfrentamento da situação tanto pessoal como profissionalmente, o medo de transmissão da COVID-19 para os familiares e a vivência do luto, foram identificados neste estudo. Conclusão: a exposição do profissional enfermeiro ao Coronavírus o leva a vulnerabilidade profissional e humana. Nesse contexto, é fundamental que haja acolhimento efetivo ao enfermeiro, tendo em vista que a base profissional é o cuidado. Então, para a qualidade na assistência as condições de trabalho precisam ser condizentes a sua função.


Objective: Objective: to describe the impacts of the COVID-19 pandemic on nurses' health. Method: in this exploratory, descriptive, qualitative study, data were obtained from semi-structured interviews of ten nurses from the Family Health Strategy in Murici, Alagoas state, and submitted to Minayo's Content Analysis technique, using Callista Roy's Theory of Adaptation as the theoretical frame of reference. Results: fear of the unknown, the need to meet the situation both personally and professionally, fear of transmitting COVID to family members, and the experience of grief were identified in this study. Conclusion: nurses' occupational exposure to the Coronavirus leads them to professional and human vulnerability. In this context, it is essential that nurses' health demands be well received and effectively met, considering that the basis of the profession is care. Accordingly, for quality care, working conditions need to be consistent with the nurses' function.


Objetivo: describir los impactos de la pandemia de COVID-19 en la salud de enfermeros. Método: se trata de un estudio exploratorio, descriptivo, cuyo enfoque es cualitativo, realizado junto a diez enfermeros de las Estrategias de Salud Familiar de Murici-Alagoas. Los datos se basaron sobre una entrevista semiestructurada y fueron sometidos a la técnica de Análisis de Conteo de Minayo. El referencial teórico utilizado fue la Teoría de la adaptación de Callista Roy. Resultados: en este estudio se identificaron el miedo a lo desconocido, la necesidad de afrontar la situación tanto personal como profesionalmente, el temor a la transmisión del COVID-19 a los familiares y la experiencia del duelo. Conclusión: la exposición profesional de los enfermeros al Coronavirus los lleva a la vulnerabilidad profesional y humana. En este contexto, es fundamental que haya una acogida eficaz al enfermero, teniendo en cuenta que la base profesional es el cuidado. Por lo tanto, para que la asistencia sea de calidad, las condiciones de trabajo deben ser adecuadas a su función.

ABSTRACT

Objetivo: La rápida progresión de la neumonía por COVID-19 puede implicar la necesidad de recurrir a sistemas de respiración asistida, como, por ejemplo, la ventilación mecánica no invasiva o la intubación endotraqueal. La introducción de herramientas que detecten la neumonía por COVID-19 puede mejorar la atención sanitaria de los pacientes. Nuestro objetivo es evaluar la eficacia y la eficiencia de la herramienta de inteligencia artificial (IA) Thoracic Care Suite de GE Healthcare (que incorpora Lunit INSIGHT CXR) para predecir la necesidad de recurrir a la respiración asistida en función de la progresión de la neumonía en la COVID-19 en radiografías torácicas consecutivas. Métodos: Se incluyeron pacientes ambulatorios con infección por SARS-CoV-2 confirmada, con hallazgos probables o indeterminados de neumonía por COVID-19 en la radiografía torácica (RXT) y que necesitaron someterse a una segunda RXT debido a la evolución clínica desfavorable. En las dos RXT se evaluaron el número de campos pulmonares afectados mediante la herramienta de IA. Resultados: Se incluyeron 114 pacientes (57,4±14,2 años, 65 -57 %- varones) de forma retrospectiva. 15 pacientes (el 13,2 %) precisaron respiración asistida. La progresión de la diseminación neumónica ≥0,5 campos pulmonares al día en comparación con el inicio de la neumonía, detectada mediante la herramienta TCS, cuadruplicó el riesgo de precisar respiración asistida. El análisis de los resultados de IA precisó 26 segundos. Conclusiones: Aplicar la herramienta de IA, Thoracic Care Suite, a la RXT de pacientes con neumonía por COVID-19 nos permite predecir la necesidad de recurrir a la respiración asistida en menos de medio minuto.

ABSTRACT

OBJECTIVE: Rapid progression of COVID-19 pneumonia may put patients at risk of requiring ventilatory support, such as non-invasive mechanical ventilation or endotracheal intubation. Implementing tools that detect COVID-19 pneumonia can improve the patient's healthcare. We aim to evaluate the efficacy and efficiency of the artificial intelligence (AI) tool GE Healthcare's Thoracic Care Suite (featuring Lunit INSIGHT CXR, TCS) to predict the ventilatory support need based on pneumonic progression of COVID-19 on consecutive chest X-rays. METHODS: Outpatients with confirmed SARS-CoV-2 infection, with chest X-ray (CXR) findings probable or indeterminate for COVID-19 pneumonia, who required a second CXR due to unfavorable clinical course, were collected. The number of affected lung fields for the two CXRs was assessed using the AI tool. RESULTS: One hundred fourteen patients (57.4 ± 14.2 years, 65 -57%- men) were retrospectively collected. Fifteen (13.2%) required ventilatory support. Progression of pneumonic extension ≥ 0.5 lung fields per day compared to pneumonia onset, detected using the TCS tool, increased the risk of requiring ventilatory support by 4-fold. Analyzing the AI output required 26 seconds of radiological time. CONCLUSIONS: Applying the AI tool, Thoracic Care Suite, to CXR of patients with COVID-19 pneumonia allows us to anticipate ventilatory support requirements requiring less than half a minute.

ABSTRACT

At a time when the COVID-19's second wave is still picking up in countries like India, a number of reports describe the potential association with a rise in the number of cases of mucormycosis, commonly known as the black fungus. This fungal infection has been around for centuries and affects those people whose immunity has been compromised due to severe health conditions. In this article, we provide a detailed overview of mucormycosis and discuss how COVID-19 could have caused a sudden spike in an otherwise rare disease in countries like India. The article discusses the various symptoms of the disease, class of people most vulnerable to this infection, preventive measures to avoid the disease, and various treatments that exist in clinical practice and research to manage the disease.

BACKGROUND: Since the beginning of the coronavirus disease 2019 (COVID-19) pandemic, studies have shown that this disease has affected the male population on a significant scale in various parts of the world, making men one of the main risk groups. OBJECTIVE: To analyze the clinical and epidemiological characteristics and experiences of illness in men with COVID-19. DESIGN AND SETTING: A mixed sequential-explanatory study with cross-sectional and exploratory-descriptive approaches. METHOD: Data was collected from a small municipality located in the central-north region of the state of Bahia, Brazil. Primary quantitative data was extracted from compulsory notification forms from 598 men. Qualitative data from individual interviews of 30 men was analyzed by the Discourse of the Collective Subject method. RESULTS: The findings identified the characterization of reports of suspected and confirmed cases of COVID-19 in men, the organization of the healthcare system, and strategies for the control and combat of COVID-19 directed towards the men of the investigated municipality. They revealed the clinical characteristics based on the collective discourse of men with COVID-19. CONCLUSION: In men, the individual experience of disease explicitly explains the clinical markers of COVID-19 expressed by the self-reported syndromic approach. Additionally, this understanding also explains the behaviors observed in their search for health care, as well as the adoption of prevention and control measures and therapies recommended by health professionals.

ABSTRACT

BACKGROUND: The role of chest radiography in COVID-19 disease has changed since the beginning of the pandemic from a diagnostic tool when microbiological resources were scarce to a different one focused on detecting and monitoring COVID-19 lung involvement. Using chest radiographs, early detection of the disease is still helpful in resource-poor environments. However, the sensitivity of a chest radiograph for diagnosing COVID-19 is modest, even for expert radiologists. In this paper, the performance of a deep learning algorithm on the first clinical encounter is evaluated and compared with a group of radiologists with different years of experience. METHODS: The algorithm uses an ensemble of four deep convolutional networks, Ensemble4Covid, trained to detect COVID-19 on frontal chest radiographs. The algorithm was tested using images from the first clinical encounter of positive and negative cases. Its performance was compared with five radiologists on a smaller test subset of patients. The algorithm's performance was also validated using the public dataset COVIDx. RESULTS: Compared to the consensus of five radiologists, the Ensemble4Covid model achieved an AUC of 0.85, whereas the radiologists achieved an AUC of 0.71. Compared with other state-of-the-art models, the performance of a single model of our ensemble achieved nonsignificant differences in the public dataset COVIDx. CONCLUSION: The results show that the use of images from the first clinical encounter significantly drops the detection performance of COVID-19. The performance of our Ensemble4Covid under these challenging conditions is considerably higher compared to a consensus of five radiologists. Artificial intelligence can be used for the fast diagnosis of COVID-19.

ABSTRACT

OBJECTIVE: to map health care actions in the organ and tissue donation process in Brazilian regions during the COVID-19 pandemic. METHODS: a mixed methods study. Data collection was performed simultaneously through an online questionnaire with 72 nurses. Descriptive statistical analysis and content analysis. RESULTS: a total of 34.7% of professionals work in the state of São Paulo. The largest number of responses was from the Southeast region. Four categories emerged. The first addresses triage care actions; the second involves guidelines for SARS-CoV-2 prevention in potential donors; the third relates to the epidemiological screening of professionals; the fourth presents the scenario of donation training in pandemic times. CONCLUSION: care actions are aimed at tracking the path taken until arrival at the hospital, assessing temperature and saturation curves, in addition to screening for signs and symptoms for SARS-CoV-2 contamination among professionals.
